Lotus Mobile Dental Hygiene respects your privacy and protects your personal health information in accordance with Ontario's Personal Health Information Protection Act (PHIPA) and applicable federal privacy law.
What we collect
When you submit a booking request, we collect your name, contact details, visit location and the information you choose to share in the form. During your appointment, we collect health history and treatment records needed to provide safe care.
How we use it
We use your information only to schedule and provide your care, communicate with you, issue receipts, submit insurance claims at your request, and meet our professional record keeping obligations. We never sell your information.
Sharing
We only share your information with your consent (for example with your dentist or insurance provider) or when required by law.
Security and retention
Records are stored securely and kept for the period required by the College of Dental Hygienists of Ontario.
